A Rough Night is the fifth episode of the fourth chapter of Trauma Center: Under the Knife 2.
Story[]
Greg reviews Derek's performance and notes that he's been making significant improvements, enough to trust him to be on call when Hope Hospital is in charge of emergency patients. Derek steels himself, but starts to doubt his own judgment. As multiple emergency patients get brought in, Derek has no choice but to focus on operating.
After an intense night managing the victims of a car accident, Greg congratulates Derek on a job well done and lets him get some well-deserved rest.
Meanwhile, the staff in Caduceus aren't doing too well. Angie's been down in the dumps since Derek's departure, and Tyler's noted that Adel's been studying even harder than usual since recovering from his operation.

Operation[]
This is a marathon operation - 8 minutes for three patients. Remember that you don't have the Healing Touch for this level.
The first patient has 2 lacerations and 2 shards of glass. Treat the superficial wounds, then open up and work on the patient's spleen. This spleen-and-ribcage operation is the same as episode 1-4, though you won't have to deal with cardiac arrest. The spleen will split with each rib fragment you extract, so treat each wound as they appear. After you've set five rib fragments, the spleen will rupture with multiple wounds as the final fragment presents itself. Extract it, treat everything else, and then fix up the ribcage to finish this patient.
The second patient has glass shards and bleeding in their left lung, same procedure should go as before on their chest. Once you open the patient you’re greeted with glass shards, bleeding, and several large lacerations. Treat these and several pockets of blood will form. Treat these and pan upwards with the magnifier. In addition to the wounds you see, there's also blood clouds that must be gelled and blood pockets that must be drained out. After you treat every other wound a few more pockets should appear at the bottom. Treat these and you’re done with the second patient.
The final patient has a windshield wiper in their right arm in addition to torn ligaments. Treat the few lacerations on the outside, saving the small ones for last. Open the patient and remove the windshield wiper, position it so that your stylus/finger ends up at the bottom left corner of the screen. Drain the blood and suture the wound. The ligaments on the other hand can be what breaks most runs for XS. This is because it’s incredibly easy to miss when pulling a ligament up, as well as suturing the ligaments together. After you fix those, close the patient up and you’re done!
