Story[]
The team finishes transferring to Caduceus USA and gets introduced to the staff and the facilities. Markus, Valerie and Elena form the basis of the Caduceus Special Disease Counteraction Team to help with Stigma treatment. While they're being oriented to the operating theaters, the chief surgeon Justin Everett comes in, asking for Kanae Tsuji to operate on his cholelithiasis. With nobody else available, the team is enlisted to work.

Operation[]
This is a gall bladder extraction, a unique operation in the series.
After the opening incision, you are introduced to gallstones and have to fragment them with the laser after locating them with the ultrasound. If a gallstone is allowed to exit the screen down the bile duct, the patient will take some vital damage and the Chain will break.
Then, start with injecting the sedative into the gall bladder before making several cuts to detach the gall bladder. In between each cut, the patient will start hemorrhaging, so you'll have to quickly drain off the blood before continuing. More gallstones will pass in the process, so make sure to scan the bile duct for new stones to fragment in between each cut. Larger stones may split into two before complete fragmentation.
After you've finished cutting out the gall bladder, you can extract it with the forceps and gallstones will stop forming. Clean out the residual bleeding with antibiotic gel, and suture the bile duct to complete the operation.

With Markus, you can excise the gall bladder before any new gallstones have the opportunity to even form. If you're rushing the extraction, you need to raise vitals first before starting, since vitals will take a hit every time the patient bleeds.
With Valerie, you don't have to worry about the vital damage at all during the extraction. After each cut, though, take a moment to scan for and eliminate any gallstones - you are not allowed to let any stones pass at all.
