Story[]
A conference is being held at the Department of Health and Human Services, discussing the recent changes in Stigma's nature. It has become contagious, and an organization has been intending to develop it as a weapon. Secretary Mendez and Deputy Secretary Marshall (who was a patient in chapter 3-5) are on the case, and the Department of Homeland Security is heading investigations.
Meanwhile, back in Caduceus USA, the staff is relieved to have the main team back. While Valerie has strengthened her resolve in the fight against Stigma, Markus suspects that the individual called "The Director" could be Professor Wilkins. The team is too doubtful about the Director's identity and resume their work in Stigma treatment.
While they are introduced to their next patient, Isabella also brings them news that Cynthia had left during the team's absence, due to arguments with the higher-ups about developing products that increase the patients' susceptibility to Stigma. Valerie is disappointed as she had hoped that she would work with her old friend to eradicate Stigma together.
After the operation, Irene approaches the team with a new mission. The fictional South American country of Culuruma, where culurium is mined and exported from, has experienced a Stigma outbreak, and requests the help of doctors experienced in Stigma treatment. Markus notes that the region is also a hotbed for guerilla activity and they could risk getting caught in civil war, but the team is willing to participate despite its danger.

Operation[]
Meet Brachion, the puzzle-like Stigma strain. Brachion has multiple arms extending from its core, and it attacks by sending toxins along its arms into the grapplers. The patient's vitals take damage once the toxin (indicated by a lit section of the arm) reaches the grappler, and you can halt the toxin's advance by pinching a segment of the arm with the forceps. If the toxin reaches a pinched segment, it will return to the core and the pinched segment will expand. With good enough timing, you can pinch a toxin-containing segment to immediately force it back to the core.
While managing the movement of toxins, you will have to inject the grapplers with purple serum so that you can excise and extract them. If a dose of toxin reaches the end of an arm without a grappler, not only will the patient take vital damage, but all missing grapplers will regrow. You must extract all Brachion grapplers to advance to the next stage of treatment.
In this stage you must extract Brachion's grapplers a total of four times to complete treatment. Note that Brachion can regrow arms of unequal length in later stages of treatment to complicate things.

Getting XS[]
Pinching the most distal segment of each arm buys you the most time in managing the movement of the toxins. Don't rush and you should be able to finish with no mistakes. A perfect operation can easily net you an XS.
