"Challenge Operations" are a series of standalone episodes unique to New Blood, providing the player a situation of elevated difficulty based off an operation of significant importance from within its chapter. Unlike the other Challenge Operations, this features a briefing, story script, and dialogue.

This episode falls after the Epilogue, within Chapter 7.

Story[]

Derek Stiles calls out to Markus and Valerie, requesting their assistance with a new VR simulation. He elaborates that the Caduceus Disease Researcher was fascinated by Stigma operations, and has devised scenarios that simulate Stigma operations. While there was only enough data to simulate Cheir and Soma operations, Valerie accepts any contribution to training more doctors in the right against Stigma. Stiles lets the doctors try out the simulation... but warns them of a "little surprise" they included.

All right then. Please try these consecutive operations designed by our director of disease research.
There's a little something hidden inside it.
A little... "something"?
This simulation has the VR data of Cheir and Soma in it.....but there's VR data from some different pathogens recorded in it as well.
Different pathogens?
Yes, I don't believe that you two would've ever seen them before.
Dr. Stiles and I will help you through them, so relax.

I understand. Let's start the simulation!

Operation[]

Welcome to the bonus challenge operation. Like the other Challenges, you have multiple patients to operate on, vitals carry forward between each patient, and a Good will break your Chain.

Open up the first patient, stitch up the wounds and a battle against a familiar pathogen begins. Say hello to Kyriaki, the first of the GUILT pathogens. It's a simple slicing pathogen like Cheir, but it lurks beneath the organ and must be found with the ultrasound and forced out with a scalpel stab. Two shots with the laser will destroy the Kyriaki. You have to fight through a couple waves of Kyriaki, then the adult will manifest with its signature asterisk-shaped slice. This one dives back into the organ when it takes laser damage, and vaporizes after taking a total of three hits.

The second patient is a Deftera operation. Each Deftera wanders about the organ, creating tumors that must be lasered off. When a red/blue pair collide, they become vulnerable and can be drained, and their movement can be influenced with antibiotic gel. After three drainages, the pair goes berserk and must be excised quickly. You first deal with one pair, but the operation will immediately spawn two pairs of Deftera when you're done. Ensure that like-colored Deftera bodies do not collide.

The third patient is where things get tricky, as you now have to contend with Soma and two pairs of Deftera. Mind the pathogens' movements as an immobilized Deftera will cover the Soma membrane, preventing you from lasering it and almost ensuring that it hardens. Between the two, Deftera is the bigger priority and should be eliminated first.

The fourth patient is a simultaneous Cheir and Kyriaki operation. You start with fighting off one Kyriaki and Cheir, then two Kyriaki with one Cheir, then a fused Cheir and adult Kyriaki. The operation will only send in the next wave after the previous one is completely eliminated. Between the two, Kyriaki is more aggressive but more fragile, and can be prioritized so that you have fewer sources of injuries to manage.

In the middle of the final wave, the game sics one last surprise on you: miniature Savato. These little bugs don't do much on their own, but if not eliminated quickly they will combine into the blue mini-Savato and cause the maximum vitals to tank. The game will sic a total of four waves of miniature Savato while you're still contending with the advanced Cheir and Kyriaki forms.
