Derek and Angie meet Blackwell, who is disappointed that his previous six GUILT strains are now treatable. He brags of his final GUILT strain which he's infected with, and is confident that it cannot be treated like the others. Angie confronts him, and Blackwell regrets that he couldn't die before he was forced to face Angie. He claims that he is neither a good father nor a life worth saving. Derek and Angie are determined to have Blackwell face his crimes, and begin treating the final GUILT.

Operation[]
Savato is the final boss of the GUILT strains. With a whole 10 minutes given to treat it, expect a difficult fight. Do not use the Healing Touch until the very end.
Savato starts its first phase resting in its web which spans the heart. It will evade everything you can use, so you have to cut away at the web with the scalpel. Doing so will melt the scalpel, rendering it temporarily unusable; fix up the patient's vitals or deal with other problems that manifest in the meantime. Savato will occasionally weave new strands, spawning mini-Savato in the process; burn them away with the laser as soon as possible to prevent a blue mini-Savato from spawning. Once the web has been completely cut away, Savato will spin a new one; repeat the process two more times to proceed.
At the second phase, Savato becomes vulnerable to laser exposure but also becomes more aggressive. It can attack by creating a laceration that spawns mini-Savato, or just cause three lacerations at once. Keep the laser on Savato and treat the other problems as they come, but avoid burning out the laser so you always have some to spare for the mini-Savato bodies. A more aggressive strategy prioritizes attacking Savato over everything else, only stopping to treat wounds and raise vitals while waiting for the laser to recharge. Once Savato's outer shell has been burned out, hit it with the scalpel. Then, repeat the process two more times to initiate the final phase.
After the second time you've stabbed Savato, Victor interrupts and presents a vial of black serum intended to destroy Savato. It can only be used once Savato enters its final phase, where it absorbs all mini-Savato and becomes a lot more sluggish. Before you inject the serum, raise the patient's vitals over 30 or its next attack will instantly kill them. With this injection, Savato goes berserk, creating nine lacerations at once and forcing the patient's vitals to 30. Derek will respond with an automatic Healing Touch to keep up with its movements, but this alone is not enough to pin down Savato. You will need to manually activate the Healing Touch a second time, which causes time to stop. Once this has happened, you can take all the time you need to fix up the lacerations Savato has made, and then finish it off with a full syringe of the serum.

If you're short on score, after burning away Savato's coat for the last time, let it linger and create more lacerations for extra Cools. Once you're satisfied, deliver the last scalpel stab to proceed with the operation. The other opportunity to rack up points is Savato's last stand, after the first injection. If you're fast enough you can suture the lacerations quickly and still have time for a dose or two of stabilizer before Savato attacks again. Alternatively, you can keep pumping stabilizer into the patient as Savato creates more lacerations, before stopping time and then stitching everything up before the finishing blow.
