Dream's End is the fourth episode of the seventh chapter of Trauma Center: Under the Knife 2.
Story[]
The raid on the HOA headquarters continues, and Mercer and Mayuzumi are nowhere to be found. One of the soldiers has located Adel, who wants to speak with Derek.
Adel confesses the resentment that he felt under Derek's tutelage. Despite everything he did in his own power, his surgical skills could not ever match up with Derek. This, on top of the betrayal he felt when he came down with the Costigar Disease, was what drove him to join the HOA and gain his artificial Healing Touch. His feelings are brought to a boil when the Neo-GUILT drives him insane, and he tries to use his Healing Touch to stab Derek with a scalpel. However, he collapses in pain and Derek has to operate to save him from his Neo-GUILT.
After a successful operation, Adel is grateful that his life was saved but is guilty over his attempt to hurt Derek. Angie and Derek reassure him that everyone has their moments of weakness, but what's truly important is the ability to overcome them and move on. Even though Adel has lost his Healing Touch, he still has the desire to save people can still become an accomplished surgeon without it.

Operation[]
This pathogen, Sige, is not as aggressive as Bythos but is rapid in speed.
After the opening incision, you'll be greeted with a few lacerations and pus pools. Fix everything up and top off the vitals before starting the fight.
Sige will start with a pair of lacerations in the lung. Stitch it up and then search for it with the ultrasound. Look for it quickly, since it will create hematoceles within the organ that can burst and deal major vital damage if unattended. Once you've locked onto Sige, force it to the organ's surface with a scalpel swipe (simple taps like how you extracted Kyriaki are not sufficient).
Sige will immediately obscure the operating field with gas, and you must disperse it by blowing into the microphone. Once the gas is cleared, patch up the gas wound (and Sige's exit wound) or the gas will spill out and obscure the screen again. While it is on the organ surface, Sige will move too fast for the eye to track and cause vital damage each time it ricochets off the operation field. It will also release pus that can obscure your actions.
Antibiotic gel, however, is effective at halting Sige's movements. A smear of gel can cause Sige to stop long enough for you to strike it with the scalpel. You will need a swipe with the scalpel rather than a tap to register a hit. Once it takes damage, Sige will submerge into the organ so you have to repeat the process. Tapping Sige's immediate location with the ultrasound will get a quick lock-on to force Sige out of the organ again.
You'll also notice that you still have access to your operating tools while you're blowing off the gas. You can smear antibiotic gel all over the organ to keep vitals up and slow Sige's attacks until you've dispersed the gas.
After a couple more strikes on Sige, you may notice that Sige will start creating false copies to confuse you. The false copies will also cause vital damage whenever they ricochet off the operating field, accelerating the vital damage. The real Sige is a bright yellow while the copies are grey. Make sure to strike the real one, as a false copy will burst in a large collection of pus and register as a mistake.
A total of five good hits should be enough to defeat Sige.

Getting XS[]
Each time you force out Sige, prioritize sealing the gas wound, then any other wounds (including pus), then an attack on Sige itself. Spread gel on the organ frequently to keep Sige's movements restricted while you're working on the other problems.
