Derek really wonders if he can afford to give up the patient-doctor interactions to transfer to Caduceus. After he performs a heart valve replacement on a child and explains to her mother that the child is at very low risk of complications, he begins reflecting on the advancements in modern medicine. Derek had lost his father to an incurable disease, and was upset by his loss. Now that he's has the doctor's perspective, he understands how hard it is to tell someone that they or their loved one cannot be saved. This hardens his resolve, and he makes the decision to join Caduceus so that he can aid their cause of saving people from incurable diseases.
Conference[]
Briefing - UtK
Today, you will be operating on Elisa Chalke, from pediatrics.
She's a very young child with valvular heart disease...
It was a tough decision, but her parents finally decided to go ahead and have the procedure done. There's only one main objective for this surgery: - Replace a valve in her heart with a synthetic one. You'll use a triple incision to excise the valve--cut three times. You'll need to be as cautious as possible, understand? Technology has allowed us to deal with all kinds of situations......but heart surgery will always rely on the doctor's skill. Angie, keep and eye on the oscillograph to aid Dr. Stiles.
I understand.
Don't let the gravity of this operation intimidate you. The conference is over. She's in your hands now, Derek.
Let's begin the operation!
Briefing - SO
Today you will be operating on Elisa Chalke, who's been in our pediatric ward.
She's a very young child with valvular heart disease...
It was a tough decision, but her parents finally decided to go ahead with the procedure. There's only one main objective for this operation: - Replace a valve in her heart with a synthetic one. You'll use a triple incision to excise the valve--cut three times. You'll need to be as cautious as possible, understand? Technology has made a lot of situations much easier for us......but heart surgery will always rely on a skillful doctor. Angie, keep an eye on the oscillograph to aid Dr. Stiles.
I understand.

Let's begin the operation!
Operation[]
This is perhaps the last instance of cardiac arrest to be found in this game. Upon starting the operation, the patient will fibrillate and flatline, prompting you to restart the heart.
After the heart has been restarted, make an incision and magnify over the valve. Drain the valve of blood, excise the faulty valve, replace it and secure with a suture. While this is a simple process, you will be regularly interrupted by the patient fibrillating again. Keep an eye on the vitals meter - should it turn yellow when it shouldn't be, withhold further treatment until either fibrillation passes or another flatline occurs. Keep the patient above 50 vitals to make abnormalities more visible.

Your biggest obstacle is fibrillation, which wastes time and can impact your bonuses. It's prone to happening after each cut on the valve. To make the time limit and minimize interruptions, activate the Healing Touch immediately after the first resuscitation. Once you've placed the artificial valve, suture across it horizontally, passing up and down over the valve, to easily get a COOL.
