GUILT Returns is the sixth episode of the third chapter of Trauma Center: Under the Knife 2.
Story[]
A few days since the previous episode, Derek and Angie still have not reconciled. The drama doesn't linger for long as an emergency alarm sounds, informing of a GUILT outbreak at Elysium.
At Elysium, as the Caduceus staff mobilizes to treat the patients, Derek and Angie find Heather and Emilio amidst the chaos. They beg for the team to save the other first. Heather loses consciousness, and Emilio convinces Derek and Angie that his previous GUILT infection gives him antibodies that can hopefully buy him time. Derek and Angie triage their patients and get to work, treating two staff members in worse condition, then Heather, then Emilio.

Operation[]
Players of the previous installments, welcome back to Kyriaki operations. This is also the game's first multi-stage operation, and you'll have 10 minutes to operate between multiple patients. The only vitals that matter towards your score are the last patient's vitals, so you can skip raising vitals before bandaging on the initial patients.
After you make the opening incision, you have several lacerations to treat, including large gashes. Close the gashes first, leave one laceration open to raise vitals, then stitch up the last laceration and prepare to face the GUILT.
Kyriaki operates by creating lacerations from within the organ itself, as evidenced by their opening attack. Stitch up the lacerations and then search for the Kyriaki with the ultrasound. Once you've locked onto the shadow, incise with the scalpel to force the Kyriaki to surface. Patch up the wounds it creates and then give it a few stings with the laser to defeat it.
Once you've been given the rundown on basic Kyriaki treatment, the game will now throw two Kyriaki bodies at once. Defeat them both, and a third mature Kyriaki will manifest, announcing its arrival with three long lacerations shaped in an asterisk. While it can be forced out like the smaller Kyriaki, a single laser burst will force it to retreat into the organ. You'll have to track it and force it out again to get another hit in. Two hits in total will destroy this Kyriaki and conclude treatment on this patient.
The second patient has roughly the same pattern to Kyriaki operations. However, once you get to the mature Kyriaki, you'll notice it looks a little different. This is the Queen Kyriaki. While it behaves no different from the standard mature Kyriaki, once you hit it with the laser, a new small Kyriaki will appear in the organ after the Queen retreats back in. Hit the Queen a second time, and Angie will notify you of a slight difference in the chiral reaction with the Queen.
Scan with the ultrasound and you will find a Kyriaki pupa - cut it out and extract it. This is the new gimmick that the Queen brings in this game. Every time the Queen retreats into the organ, it will lay a few pupae within the organ, and if left unattended they will hatch into new Kyriaki whenever enough time passes or the Queen is hit with the laser. This pupa is the only one you will see in this patient, so get the last hit in on the Queen, burn off any remaining Kyriaki bodies, and close up this patient.
The third patient is Heather, who has a full-scale Queen Kyriaki infection. When the Queen makes its entrance there will be a pupa to find before you damage it. The next times it dives in it will spawn three pupae each time. The Queen will take a total of four laser hits before it finally goes down.
