Operation[]
Meet Paraskevi. It will manifest in Cybil's small intestine after treating the initial lacerations. This worm-like GUILT meanders through the body's systems, aiming for the heart where it can deliver instant death.
The GUILT is initially large and rather slow, but also too big to extract. You have to stun it by striking its tail with the laser, and then slice it in half. The halved Paraskevi will proceed to make two more lacerations, but are still too big to extract. Once it's been split into a sixteenth of its original length (four cuts), then will the Paraskevi be small enough for an extraction.
If any Paraskevi body is left untouched for long enough, it will burrow into the organ, and surface in its next destination. From the small intestine, the Paraskevi will advance to the stomach, liver, and then the heart. Once it burrows into the heart it's an immediate game over, so stun all Paraskevi bodies at first opportunity. In Second Opinion, any Paraskevi about to burrow into the organ will rapidly wag its tail; this visual cue is absent in Under the Knife.
A common strategy is to focus down one piece of the Paraskevi at a time, minimizing the number of fragments to manage in the operating field. If rank is of no importance, you can allow the one half of the Paraskevi to escape into the next organ so you can continue focusing on the half you're extracting. Make sure to not let too many fragments escape, as any Paraskevi near or on the heart, no matter how small, leaves very little room for error.
