Insecure Smiles is the first Endoscopy level in Trauma Team.
Opening[]
Tomoe and Hanzou run into Maria on the roof of Resurgam. Maria lands her helicopter and reminds Tomoe about the day's operation on Joshua Cunningham. She offers to support Tomoe, but she refuses and tells Maria to rest. Maria decides to support anyway, with Tomoe deciding she cannot refuse her kindness. She asks Hanzou to make the preparations. As Hanzou enters the operating room, Maria is stunned by the sight of Tomoe's custom endoscope. Hanzou points out its special design, and Tomoe declares it to be "the path of honor" to make sure she uses the best tools. While Maria is still shocked, Tomoe begins the briefing.
Operation[]
This operation serves as a tutorial for the endoscopy function. The game will explain to you how to move the tool around in place, forward, and back. Once this is complete, the game will ask you to move forward to find the affected areas. When you find the first blood pool, it will instruct you how to use the drain to cure it. You will then move past the area where it was. After moving far enough forward, you will find a hemorrhage, and the game will teach you to use the hemostatic forceps. You will be unable to use the tool because there is a blood pool in the way; drain it. The patient's vitals will be lowering; the game will tell you when an operation ends, and ask you to use the stabilizer to keep vitals up. Keep going and the third area, an ulcer, will be visible. You must treat it by using the syringe to inject medicine. After this, the game will tell you to look for more affected areas on the radar. Follow it to find the next ulcer, which is treated the same way as the first. After the first hidden one, you will reach the stomach, and you will have to wait carefully to make sure the dilating opening is wide enough to pass through. A number of hemorrhages, blood pools, and ulcers will appear; keep treating them as before. After treating the first set of affected areas, the vitals are still not stable; the game will give advanced directions for using the radar. This will enable you to find another hidden ulcer. Reveal and treat it and the operation is complete.
Ending[]
Once the operation is finished, Tomoe expresses confidence that Joshua will recover. Maria eagerly cheers her on, and Tomoe modestly thanks her for her support. Maria realizes she needs to contact his mother and let her know how it went. Tomoe reminds her to tell Gabriel too. Maria laughs about how much parents worry about their children, and even the workers at her old orphanage bother her sometimes. Tomoe brings up how she lost her mother when she was little, and she also had an argument with her father before leaving. Maria apologizes, and Tomoe reassures her. Maria tries to boost her confidence in her surgical skills. Right before leaving to eat, Maria and Tomoe are bothered by other workers asking favors.
