Story[]
The team finally arrives at Master Vakhushti's research room, who welcomes them due to their ties with Cynthia and Stigma. Markus resolves to eradicate Stigma, while Vakhushti hails it as "new life" for a new world that would be founded on the chaos it causes. Markus notes that Vakhushti is heavily infected and prepares to operate. Over the radio, the rest of Caduceus lends their aid, and it's now a collective effort to defeat the final strain of Stigma and end it once and for all.

Operation[]
Cardia is the final Stigma strain and the final boss of this game. Do not use your Healing Touch until the very end of the operation.
You start by working on the membrane that Cardia has over the heart. Gel pieces of the membrane to soften it so you can excise and extract them. You can do it one square at a time for maximum chain, or remove multiple squares' worth of membrane in a single block to save time. There are a few glowing red squares that must be removed first - if Cardia touches these squares the patient's maximum vitals will decrease. You cannot remove any pieces of membrane that Cardia is standing on, but Cardia occasionally teleports on and off the operating field to give you a window of opportunity to extract the last piece of membrane.
After fully extracting the membrane, Cardia will create a new one, undergoing stress each time the membrane reforms. You must remove the membrane a total of three times to advance to the next stage.
Once Cardia sheds its shell, the real battle begins. It becomes vulnerable to the laser, but also turns a lot more mobile and attacks more aggressively. It will periodically create lacerations (sometimes one that is haemorrhaging) before performing its signature attack. It will dart across the heart and deposit several red tumors before releasing a purple slowly-expanding ripple. If the ripple touches a tumor it will rupture and damage the patient's vitals, so extract all the tumors before that can even happen. Cardia stays still while radiating the ripple, giving you a good opportunity to deliver a lot of laser damage to it.
When Cardia's taken two OK's worth of laser damage, it gets even more aggressive, sometimes creating five or six lacerations in quick succession. Keep the patient's vitals up whenever you can to minimize risk of sudden death.
When you've done the third OK of laser to Cardia, it will go berserk and deposit a large ring of tumors on the heart. It will then move to the center of the screen and radiate its ripple. This is the moment to use the Healing Touch. As Markus, you will be given enough time to extract the ring of tumors before blasting Cardia into oblivion. As Valerie, the patient will be able to tank the tumor explosion - your Chain will break, but you're already at the end of the operation.

Getting XS[]
Don't rush. The 150 Chain bonus will come naturally as you work through the operation. Extract the membrane carefully and try to avoid getting a Bad on your suturing technique in the second half of the operation.
