Secret Patient is the first episode of the seventh chapter of Trauma Center: Under the Knife 2.
Story[]
The aftermath of the defeat of Delphi is bittersweet for Caduceus. While the overt medical terrorism threat has come to an end, the Hands of Asclepius has essentially overshadowed Caduceus due to its contributions to ending Delphi. Meanwhile, Sidney and Hoffman discuss a confidential operation that they've enlisted Derek's help for. Once night falls and the patient is being flown in, he briefs Derek on the situation: The patient is Annie Fisher, a famous tennis player whose condition abruptly worsened, and the concerning factor is that chiral reactions were positive, and its patterns didn't match any known GUILT strains. It's up to Derek's skills and wits to treat this new GUILT strain.
After a successful operation, Derek asks Annie about any details that might have led to her sudden change in condition. She recalls taking special medication that her coach suggested to her, and while its effects on her body were tremendous, she started doubting herself on whether it's a legal performance enhancement drug. After racking her brains, the drug she took originated from Acropolis Pharmaceutical, a grave revelation for the Caduceus team.

Operation[]
After you've made the opening incision, you've got a few lacerations to treat on the small intestine. Nudge the vitals up before finishing the last laceration and prepare for the battle.
This is Bythos, a Neo-GUILT. It's also one of the most aggressive pathogens in this game.
You damage Bythos with laser exposure, but it will take a few seconds before it will register as a hit. While it's going about the organ it will also create lacerations, and the damage from the lacerations being created on top of lingering vital loss makes treating these lacerations a priority. Work quickly so you can get back to lasering Bythos before it attacks again.
Once you've done enough damage to Bythos, it will split apart and release several circulating spores around the core. Pick up the core with the forceps and extract it before the Bythos shell merges. However, if the core touches a shell fragment or any of its spores, this counts as a Miss and Bythos will regenerate. Blue spores orbit the core in a fixed radius, while green spores will bounce around and home in on the core while you've picked it up. Brown spores, released later in the operation, will also rotate around Bythos but move faster than blue ones. Extracting the core will count as a proper "hit" on Bythos.
Later in the operation, Bythos will also start using another attack where it warps across the organ. The point where it disappeared is where it has created a hematocele. Incise, drain and suture before the hematocele bursts.
After five successful core extractions, the patient will convulse one last time before Bythos is vanquished.

Getting XS[]
Try to attack Bythos while it's at the lower-right corner of the screen so that core extraction is a lot easier. The petri dish to receive the core will cover most of the spores that will otherwise obstruct extraction.
If you have two styluses, while you're holding onto the core with the forceps, get the other stylus and press on the dish, then release the first stylus. You'll warp the core past all the obstacles and get a quick and easy extraction.
