Story[]
As tensions cool down, Moreno comes in with three patients in tow: One of them is the captured Ramsey Ramirez, and the other two are foreigners - one heavily injured and the other unconscious. The doctors recognize them as their former captors. Leland regrets coming to Culuruma, while Markus quickly checks Kidman's body and pronounces him dead. The team notes that both Ramirez and Leland have ruptured livers, and decides to harvest the other former captor's liver for transplant.
With the two survivors saved, Moreno takes Ramirez away for questioning, while the team takes Leland into custody so that he can give more information on the source of Stigma attacks and Professor Wilkens's location.

Operation[]
This operation involves two back-to-back liver transplants. You start with the patient at a low vitals cap, and after positioning the liver lobe, you have to shrink each vessel by injecting a constrictor. Then, make an incision into the vessel, drain away the blood, stretch the vessel of the matching color from the liver to the incision site, and suture. With each successful connection of the vessels, your vitals cap increases by 10, capping at 80 for each transplant patient. Start with the blue vein, then work to the green portal vein, red artery, and yellow bile duct.
Well, that would be a simple operation if it weren't for one twist. The moment you connect the vein you find that Kidman's liver is infested with Stigma! You now have to deal with Cheir while finishing the transplant, so you have the option of either eradicating the Cheir with a lower vitals cap, or focus on the transplant to push your vitals cap first at the risk of Cheir attacking unimpeded. Since this is a split liver, the Stigma has less room to create wounds. This Cheir has also undergone a mutation - the second wave will include a Cheir body that can create tumors within the organ, which can be extracted like any other tumor.
Once you've completed this first transplant, it's safe to say the second transplant will present a similar challenge. This half of the liver contains a simultaneous Cheir and Onyx infection. Treating Onyx will take priority as letting it attack a single time can easily kill the patient. The smaller liver size also means that you have a smaller area to search for Onyx.

Getting XS[]
If you're very good with Stigma treatment, you can devote your efforts to defeating Cheir before finishing the transplant at your leisure. You can use the laser to push Cheir off the bounds of the liver and deny them opportunities to attack. Alternatively, defeat only one Cheir body so that you have the breathing room to finish the transplant and raise the vitals cap.
On the second patient, always defeat Onyx first - the other lone Cheir body poses little threat while you're focused on Onyx.
