Sinner is the fifth episode of the second chapter of Trauma Center: Under the Knife 2.
Story[]
Sidney speaks of Elysium, a center devoted to treating PGS patients. However, with the outbreak of PGS their patient load is beyond their capacity, so Caduceus has accepted the patients requiring more intense care. One of these patients is Emilio Juarez, who is one of the Sinners that Derek rescued at the end of Under the Knife.
Derek and Angie pay Emilio a visit, and his treating nurse, Heather Ross, offers to interpret. Emilio expresses his gratitude for all that Derek had done three years ago. But now, with his advanced PGS ravaging his liver from the high concentration of GUILT he was exposed to, he requires high-quality treatment to be cured.
A successful operation stabilizes Emilio's condition, but the extent of his liver damage means that he will need a transplant. Heather continues sorting things out at Elysium to arrange for a proper transplant.

Operation[]
Once you begin the operation, Emilio will undergo cardiac arrest and will need immediate resuscitation. Once he's recovered, inject the sedative to relieve toxicosis and begin work on the liver.
First off is the massive fissure in the middle of the organ. Drain away all the blood, affix artificial membranes across the fissure, and secure with antibiotic gel. Do this quickly, or blood pools will reform while you're working and cause the membranes to fall off.
Once you've affixed the membranes successfully, the patient's condition will deteriorate and it will haemorrhage again. Now, in addition to sealing the giant wound, you also have to locate a total of six blood pockets around the rest of the liver, cut them out, and drain them. In the middle of all this, you also have to contend with further instances of cardiac arrest that can waste more of your time.
If you've managed to pack the giant fissure first, rest assured that the membranes will no longer fall off once everything is secured with gel.
Finish treating all the wounds and the patient will stabilize, completing the operation.

Getting XS[]
The moment you correct the cardiac arrest at the start of this operation, start Healing Touch immediately. It should last long enough for you to work through the entire operation without any worry for the membranes falling out or recurrence of cardiac arrest, while also netting you the time bonus with room for error. Bear in mind that vitals will drop as you cut out blood pockets so don't forget to pump up vitals to stay out of the red.
If you're attempting to XS without the Healing Touch, you have a lot less room for error and have to work very quickly. Start with fixing the large fissure over anything else. Then, once it's re-bled, relieve toxicosis and push vitals up to 50. Take out the blood pockets on the right, nudge it back over 20, and then treat the blood pockets on the left. If cardiac arrest happens, you have no choice but to reset, so wait out the fibrillation warning.
