Story[]
Despite having dodged the night shift, Valerie and Markus find themselves snowed in at the hospital. As Markus attempts to turn in for the night, Valerie is concerned about a Concordia investigator who's due to show up and needs their immediate attention. He isn't worried about it due to the heavy weather. Just then, they receive an emergency call — a man's been shot in the chest with a rifle, and is rapidly losing consciousness from blood loss. With a few minutes until his arrival, Markus and Valerie prepare for surgery.
As the patient arrives, the caller introduces herself as Elena Salazar, a nurse from Concordia Medical Institute. With nobody else to assist, she volunteers herself to help with the procedure. After a successful surgery, Valerie is appreciative of Elena's skills and welcomes her to the team. Markus, however, is a little more cautious, and wants to know of Wilkins's motive for sending Elena down. Elena reveals that she's also come down to Montgomery to ask for Markus to operate on her implant that she's received years ago.

Operation[]
You're now introduced to the concept of low vital caps. This patient cannot be raised past 30 vitals no matter how much stabilizer you pump into him, so work with caution.
Start with the external wounds — drain the blood, suture the lacerations, and patch up the entry wound. Once done, make the incision and get to operating on his heart.
There's blood all across the pericardium, so drain it before doing anything else. This will reveal where the bullet is — make the incision to expose the bullet and begin extraction.
At this point, the patient undergoes cardiac arrest as their vital cap drops to 10. You can't resuscitate him just yet as the rest of the bullet is still lodged in the heart. Widen the incision with the scalpel and extract the bullet, draining any blood pools that form on the way. Do this quickly, as vitals will still be draining and another blood pool will form if you take too long.
Now that the bullet's fully extracted, patch up the incision and resuscitate the patient. Follow the guide and position the Wiimote and Nunchuck, then press Z and B when the voltage meter is in the right position. Once resuscitation is complete, close up the patient and finish the operation.

Getting XS[]
The time bonus is extremely tight, so work quickly. The main obstacle here is the defibrillator - master the use of it to resuscitate the patient in one shot, and you should be able to finish within the stringent time limit.
