Amy's condition deteriorates and she needs emergency surgery. Tyler, however, has no confidence in Amy's survival, after seeing her suffer for weeks while Caduceus's attempts to treat her have been futile. His point is almost proven when the GUILT is shown to have metastasized to her lungs, and even Dr. Clarks is reluctant to try. As Derek and Tyler argue over needing to operate, Amy speaks up, voicing her determination to live. Tyler is greatly moved by her words and steps down, letting Derek take the reins and briefing him on the Deftera strain in the process.
Tyler is ingratiated to Derek for finally saving his sister, and is willing to subject himself to whatever consequence Derek would find appropriate for his euthanasia research. Derek instead urges Tyler to edit his thesis and use his euthanasia findings to aid in saving lives.

Operation[]
This operation introduces the Deftera GUILT strain.
You first operate on Amy's stomach, which has only one Deftera pair and serves as a tutorial to its mechanics. Burn off any tumors created by the Deftera bodies and manage the patient until the Deftera bodies collide. Drain them while they're consuming each other, repeat this process two more times, and then quickly excise the pair when they go berserk.
The second half of the operation moves to Amy's lung, where there are two pairs of Deftera. This is where the operation becomes significantly more difficult. Deftera of the same color coming into contact with each other will create large amounts of tumors and damage the patient's vitals. A stray Deftera body touching a fused pair can split them apart and interrupt treatment.
Deftera's movement is random, but application of antibiotic gel can alter their movement (with slight vital damage) to make the operation less luck-based. Focus on extracting one Deftera pair at a time so that the tumor buildup doesn't spiral out of control.

There's a significant Chain threshold here. If you worked too quickly and feel you won't have enough, leave a pair of Deftera untreated so that they can spawn more tumors to burn off. As long as you make no mistakes the XS should be easily obtained.
