Story[]
Markus explains Elena's history — she has a rare genetic immune disorder that affects her pancreas. It can be treated with the help of a high-efficiency pump implant that goes on the pancreas, but it only lasts 20 years and Elena's due for a replacement. Since Markus previously operated on Elena, she wants him to repair the pump, and even got Concordia to mail the parts straight to Montgomery. Markus elaborates that the operation is complex, and they need to operate quickly once the preliminary test results are back.
While waiting, Valerie encounters Elena outside the hospital. She's impressed that Elena's managed to attain her International Nursing License despite having to deal with her genetic disorder. Elena's grateful for being alive and hopes to achieve more. They admire the Northern Lights as it appears in the night sky.
Meanwhile, Markus is meditating in preparation for the operation, only to be interrupted by a call from Wilkens. Wlikens asks for Markus's assistance and his capabilities, but not just to save Elena. He tells Markus that "Stigma has awakened", and he's been affected by it. He reminds that Markus cannot run away from taking responsibility for it.
With this foreboding piece of news weighing on him, Markus prepares for surgery. As things go awry, he activates his Healing Touch to save the day, but collapses from exhaustion right afterwards. Valerie takes over to finish up the operation as he's sent to the doctor's quarters to rest. After a successful operation, Valerie expresses her interest in learning about the Healing Touch, though Markus isn't as enthusiastic, only telling her to "try harder".

You can only play as Markus for this operation, which introduces the Healing Touch. Up till now, there has been no gameplay difference between selecting either doctor.
After the opening incision, you'll encounter minor hemorrhaging. This can be cleansed with application of antibiotic gel, but if left unattended for too long, they form into blood pools which must be drained. The intensifying of hemorrhages will also break your Chain.
Once the operating field is cleared up, disable the control chips with the laser, cut them out with the scalpel and extract with the forceps. Then, replace the new chips from the tray on the left with the forceps. Be wary of any more bleeding that happens while you're working.
After three chips are installed, massive hemorrhaging will occur. As much as you would try and drain the blood pools, they won't stop. Don't forget about minor hemorrhaging that happens in the interim, and spread the occasional smear of antibiotic gel to keep your Chain from breaking. After enough time passes, Markus will activate his Healing Touch automatically.
Once the Healing Touch kicks in, it will remain in effect until you finish the operation. Drain off the last of the blood and install the final chip, and the operation will end immediately as he collapses in exhaustion.

Getting XS[]
The "blood pools" bonus also counts the automatic 6 blood pools that appear, and any new ones that automatically form while you're draining the blood until the Healing Touch kicks in. Since you have to drain 12 more blood pools before the Healing Touch triggers and the operation can be completed, you essentially have no room for error when dealing with hemorrhages. Treat all hemorrhaging on sight by spreading antibiotic gel between steps, and waste no time to finish the operation once Markus activates his Healing Touch.
