Story[]
The situation with Stigma grows grim as an international syndicate has been using it to terrorize the world. The Caduceus team is being deployed to aid a strike force that's going to attack a hideout. Before they set off, Dr. Cromwell hands them a new drug that can inhibit the Stigma's activity.
At the hideout, the FBI agents commence their raid. Blows are exchanged with the remnants of Parnassus, who are now unrestrained with their use of biological weapons. As the Caduceus team treats three infected soldiers, the FBI successfully captures the hideout and arrests all remaining members. Among these arrests, the team finally finds the missing Professor Wilkens, though he is not acting like himself.

Operation[]
Welcome to another Stigma marathon operation. This time the game pulls out all the stops, and you're expected to handle two strains on a single patient. This can become one of the most difficult operations in the main story.
The first patient is a Brachion infection. No differences from the previous Brachion surgeries, but know that you do not have as much leisure with your time. Treat it quickly as the full brunt of the operation kicks in for the next patients.
The second patient is a simultaneous Soma and Cheir infection. This also introduces the third variant of Cheir which can create large gashes that need drainage before closure. Between the two pathogens, Cheir takes priority. Make sure you don't overuse and burn out your laser because you'll always need some to burn off a Soma tumor. Soma takes fewer successful drainages to defeat than usual in this operation.
The third patient seems like a lone Soma infection, but pick up the ultrasound and scan the center of the organ - you'll find Onyx hiding as well. If you don't find it, Onyx will make itself known with its usual devastating attack. It's imperative that you treat Onyx first, but you also have to keep watch for Soma's tumors so that it has no chance to harden. Luck may occasionally play a big factor in determining the Stigma's movements for a successful operation.

Getting XS[]
You have very little time to find Onyx in Hard difficulty. Once you've opened up the third patient, scan for it immediately and start fighting it, and use every trick you have in the book to ease the search for it. Due to the vital cap, a single strike from Onyx is essentially a game over.
