Story[]
Markus reviews the patient's progress and concludes that her Stigma infection has been successfully treated. Irene gets Cromwell to report the results of the pathogen study. He reveals that Stigma is unlike any known organism, and because it's very difficult to replicate, it's difficult to determine Stigma's mode of infection.
The staff then receive an emergency call: The Deputy Secretary has collapsed, and he may have an intra-cranial hemorrhage. While the team awaits CAT scan results, they review his medical history: He's had deep brain stimulation to treat his dystonia, and he also has a hippocampus implant to improve his memory. This implant was also created by Columba & Cornix, the same company that produced Elena's implant. With the CT results in, they deduce he's bleeding from ruptured brain aneurysms, and get to work.
After the operation, Markus reflects on the trends between the recent patients. A lot of the past Stigma patients had a medical device or received artificial implants, and even the mouse he experimented on which developed Stigma was using artificial blood. While he ponders a connection between all of them, Irene comes in to check on the Secretary's condition. Markus assures that he's on the road to recovery; Irene notices that Markus may be hiding something, and encourages that he open up to the staff.

Operation[]
This operation introduces aneurysms. Since you're working on the brain, you start and end right at the operating field, without any opening incision or a closing suture.
The aneurysm treatment is as follows: sedate the bulge by injecting the brown sedative, excise with the scalpel, extract with the forceps, drain off the blood, then close the arteries and suture.
While this seems very simple, the challenge of the operation is when you have to handle multiple aneurysms at once. If you spend too much effort focusing on one aneurysm, the others have a chance to swell and burst, causing vital damage and breaking the Chain. Once you've shrunk and cut an aneurysm, you no longer have to worry of that sedative wearing off or the aneurysm bursting again, so work to get every aneurysm fully shrunk and cut before anything else. You may need to spread a single syringe of sedative across multiple aneurysms at once.
The moment you close off some arteries, other aneurysms may manifest. Make sure to quickly scan for any new ones to work on.
The final wave gives you 4 aneurysms to work on at once. Due to the low vitals cap, having all 4 burst at once is almost certain to cause a game over. There is no harm using the Healing Touch to assist with this final phase.

Getting XS[]
Due to the strict conditions, you essentially have to use your Healing Touch otherwise you can't get a high enough score to reach the threshold even with a perfect operation. During the second phase when you start dealing with aneurysms in pairs, shrink an aneurysm until the scalpel guide appears for the "OK", but let the sedative wear off as you shrink and treat the other one. You'll get more "OKs" without your Chain breaking or going into great risk of aneurysms rupturing, giving you just enough score to claim the XS. Markus's Healing Touch is ideal for the final phase as you get enough time to shrink every aneurysm on screen before the sedative wears off. If you're playing Valerie, her Healing Touch may not be as ideal for time but you prevent a lot of vital loss due to bleeding from any cut aneurysms.
