The HOA is the first episode of the fifth chapter of Trauma Center: Under the Knife 2.
Story[]
Patrick Mercer, the leader of the Hands of Asclepius, is introduced, discussing the Asclepius Project with Hoffman. He outlines that he aims to improve the training of doctors worldwide so that practitioners can be brought up to Caduceus standard quickly and regularly, and is recruiting doctors to take part. Hoffman and Sidney are appreciative of his ambitions but with the ongoing resurgence of GUILT they are unable to spare much manpower.
Meanwhile, Derek catches up with his colleagues back in Caduceus, but also learns that Adel has not handled the outcome of his operation well. Adel had hoped that Derek, with his Healing Touch, would be the one who could save him from the Costigar Disease, but as Tyler could not bring himself to explain how Derek lost the Touch, Adel felt betrayed by Derek's failure. Derek eventually runs into Mercer who outlines his Asclepius Project and extends an offer to Derek, who firmly declines it. The conversation gets interrupted with the arrival of an emergency PGS patient. Derek is entrusted to work on it, and Mercer takes the opportunity to observe his skills. Mercer is impressed by Derek's skills, and lets Sidney and Hoffman know that he has already started receiving letters from Delphi remnants, confirming their return.
Derek approaches Adel and attempts to apologize for failing to save him, but Adel has already decided to resign from Caduceus to participate in the Project.

Operation[]
A surprisingly tedious operation that doesn't add in any new mechanics.
After the opening incision, you've got three blood pools. Drain them off, extract the tumors underneath, then burn off the polyps and seal everything up. As you pick up the magnifier, the patient will undergo cardiac arrest and must be defibrillated. After recovery, you've got four more tumors to extract - one near the pylorus at the left and three near the antrum on the right. Be aware that the patient can undergo cardiac arrest mid-operation and force you to halt your actions.
After these tumors are extracted, a convulsion will happen and more tumors will have appeared beneath the organ. Extract one and the operation gimmick will trigger, denying you access to the drain for a short time. While you wait it out, manage the rest of the operation by sustaining vitals or picking out the other tumors. Once you regain access to the drain, extract the five tumors and the PGS will be cured.

Getting XS[]
The tricky bit here is the blood pools bonus, because there's so many of them that will appear mid-operation. Tumors will spawn them, and polyps will create them as you burn them off. Blood pools will form faster near existing blood pools, so make it a priority to drain off any and all blood before proceeding with each step. Great points to use the Healing Touch are either in the second or final wave of tumors so that you can extract all tumors in a wave in one fell swoop, avoiding further bleeding.
