Opening[]
At the Tachibana home, Tomoe returns in time to see her father, who reports his lung problems. He tells her that the question of clan succession has been raised, and it can only be Tomoe's husband. Among the disapproving murmurs of the clan in anticipation of her refusal, Esha and Gabriel burst in. The clan shouts in fury and tries to seize them, but Master Tachibana stops them, asking if the two will take Tomoe back. Esha affirms this, and refuses to let Tomoe be married against her will. Before anyone can do anything, Master Tachibana collapses. Amongst the clan's panic, Tomoe begins giving orders to tend him, and examines him with Esha and Gabriel. They deduce that Master Tachibana has tension pneumothorax, but his skin is too tough for needles to penetrate. Hanzou arrives and slashes him with his blade, allowing the air to escape. Tomoe, stating it to be a ruptured bulla, makes preparations for an immediate operation.
Operation[]
For this operation, a 10-minute time limit is introduced. Otherwise, this shouldn't affect your performance. Search the lung for the ruptured bulla. You will have to constantly treat hemorrhaging areas along the way. This patient's vitals are low, so stabilize them quickly. The rupture will be accompanied by severe hemorrhaging. Attempt to spray the hole with fibrin glue. It will bleed again and be irreparable, so you must look around for the source. Continue to treat any bleeding you find along the way. Soon you will find a thrombus, which is causing the problem. This must be treated the same way tumors are treated. It will hemorrhage once removed, so be prepared to stop it. Now you can go back to the ruptured area, stop the hemorrhaging, and spray it with the glue. At this point, the operation is complete.

Master Tachibana wakes up and asks for Tomoe. Hanzou reports that she dazzled the clan elders with her performance. He also passes on a message: that Tomoe intends to stay in America. Master Tachibana sends Hanzou with her. Tomoe, Esha, and Gabriel return to America on the clan's private plane. They discuss Tomoe leaving Hanzou behind, unaware that he is boarding the plane. He confronts them, startling them. Tomoe is surprised as well, but glad to have him.
