The Simplest Truth is the fourth Diagnosis level in Trauma Team.
Opening[]
Gabriel is talking on the phone with Esha about a new epidemic of some mysterious disease. She notices he sounds tired and asks if he's okay. He answers that he's just fed up and hangs up. Soon, Maria comes in with a request that he examine his son.
Diagnosis[]
Phase 1[]
This starts off with a bit of an awkward exchange, mostly Joshua trying to tell his father he's okay, and Gabriel insisting that he stay and be checked out. Initially, questioning him, using the stethoscope or EKG, or performing a visual exam will not reveal anything. Joshua will then burst out coughing, enabling you to redo all the exams. Nothing will be revealed when you question him, but the stethoscope will detect the following: "Tachycardia" and "Tachypnea". Joshua will cough again into his hand, but will cover it up. You need to expose his hand by making him lift up his shirt; blood spots will appear on it. By doing this, the following symptom is revealed: "Hematemesis". Next up is an X-ray. The following symptom can be obtained here: "Caved-In Gastric Wall". After this, we can question Joshua again. The symptoms obtained here are: "Stomachache" and "Lack of Appetite". We can make a diagnosis now: "Gastric Ulcer". Due to Joshua's recent endoscopy, we believe there may be more to his condition.
Phase 2[]
- "Sudden relapse of gastric ulcers has been confirmed. Continuing diagnosis to determine cause of the relapse."
Even after his diagnosis is made, Joshua is still reluctant to talk. We run a diagnostic test, which reveals: "High Serum Amylase Level", "High Serum Lipase Level", and "High Elastase Level". We run another X-ray and a CT scan. Checking these reveals: "Abnormal Bowel Gas in Intestine" and "Swollen Pancreas". Return to the exam room and discuss your findings. Joshua will break down and allow you to question him. This will reveal the following: "Back Pain" and "Nausea". Performing a visual exam will yield the following: "Blood Spots on Left Abdomen" and "Blood Spots Near Navel". This will enable you to make your final diagnosis: "Acute Pancreatitis". Since there is no known link to the ulcers, diagnosis must continue.
Phase 3[]
- "Confirmed onset of pancreatitis, unrelated to the gastric ulcers. Diagnosis will continue."
Redo the questioning and diagnostic test. Questioning will reveal the following: "Polydipsia and Polyuria". A diagnostic will yield: "High Serum Calcium Level", "Low Serum Phosphorus Level", "High Serum PRL Level", "High Serum Gastrin Level" and "High Intact-PTH Level". Try to put all the pieces together in your office. Do an X-ray and scintigraphy; these will reveal: "Subperiosteal Bone Resorption" and "Parathyroid Adenoma". When you return, questioning Joshua will note the following symptoms: "Visual Field Loss" and "Headache". An MRI will be performed; doing so will yield: "Tumor Shadow on Pituitary Gland". Another CT scan will have to be done. Examining this will reveal: "Tumor Shadows on Pancreas". After this, we can make our final diagnosis: "Wermer's Syndrome".
Ending[]
Soon after Gabe completes Joshua's diagnosis, Maria rushes into his office, informing him that Joshua's conditions have gotten worse due to the Wermer's syndrome. She then tells Gabe to be present at Joshua's surgery, but Gabe simply tells her that being there won't change anything. A disgusted Maria calls him a disgrace & leaves the office. After that, RONI tells Gabe that what he said was incorrect, saying that his words don't match with his emotions. At that moment, Gabe loses his temper & begins trashing his office, lamenting how he can't save Joshua himself & that nothing he does matters. Moments later, Gabe finally calms down & considers himself the worst father ever. RONI (despite being badly damaged) then asks Gabe if the work he does is pointless & if the smiles on patients' faces mean nothing to him. At that moment, Gabe laughs that a robot has managed to talk sense into him. In the end, Gabe leaves his office to go to Joshua's surgery, determined not to let his son die--not as a doctor or as his father.
