The soundtrack of Trauma Center: Under the Knife (Catalog number: SRIN-1088) was released on October 5th, 2011 by SuperSweep Co., Ltd. at a price of 2000 yen (+tax). It contains all music used in the game, in both Nintendo DS instruments and (for some songs) the composers' original intended instruments (officially known as "Original Sound Version" (オリジナル音源バージョン), later used in Trauma Center: Second Opinion).
The soundtrack is also included in the game data of the Second Opinion soundtrack, but with a different order, and goes unused. Unlike the (actually used) BGM audio files, the soundtrack audio files fade out at the end and does not loop.

Trivia
- While Shoji Meguro and Kenichi Kikkawa used the traditional method of converting music to the DS's audio chip after it has been composed on another platform, Kenichi Tsuchiya composed his music directly on DS hardware. Therefore, because Tsuchiya's music consists only as a mix of samples sequenced directly on the DS's PCM/ADPCM channels and of PSG sound, they do not appear in the "Original Sound Versions".
- Since there isn't a soundtrack for Trauma Center: Second Opinion, the remastered version of Tsuchiya's music and 5 newly-added music in SO (Chapter Z, Chapter 6, the Episode Select screen, Mutated Savato, new opening movie) are never released on a soundtrack.
