Operation Strategy[]
Welcome to the postgame. The X missions are unlocked after you complete the main story, and feature extremely hard versions of the GUILT you've been battling to put your surgery skills to the ultimate test. The "Extreme" difficulty is one notch above "Hard", so only consider tackling these missions if you're already comfortable with GUILT operations on Hard difficulty.
After the initial pre-GUILT wounds, you will have to deal with three Kyriaki bodies at once. Each Kyriaki will also take 3 hits of laser to defeat. After you've defeated the first two, two more will spawn within the organ, making a total of five. When you've cleaned out the waves of Kyriaki, the mature one will spawn alongside an immature Kyriaki.
If you're playing Under the Knife, this mission is infamously difficult due to the vital damage done by Kyriaki's lacerations. The pathogen is also extremely aggressive here, so if you're suturing each laceration as they come you may not have enough breathing room to raise vitals. Instead, focus entirely on finding and taking out the Kyriaki bodies, and use the long-lasting antibiotic gel to arrest the bleeding and buy you the windows of opportunity to keep vitals up. The game can only handle a maximum number of lacerations on screen, so the risky choice of keeping lacerations unsutured means that Kyriaki will eventually stop attacking.
If you're on Second Opinion, Kyriaki is made a little more tolerable. You have enough breathing room to suture every laceration on sight and enough reprieve to keep vitals up in between treatment steps. It also helps to spread antibiotic gel across each laceration before working to suture them. Make sure to raise vitals to at least 50 before you coax out the first and final waves of Kyriaki.
