You've got more initial lacerations and inflammations to treat, and Sige will open the fight with four lacerations. Hematoceles also occur more frequently while Sige is under the organ, and from the get-go Sige will create false copies to disorient you. Sige now takes six slices to defeat, and as its health goes down it will generate even more false copies, capping at four false Siges at its last legs.
Otherwise, there are no significant new mechanics from Sige's introductory operation, so practice on episode 7-4 to refine your skills.

Getting XS[]
Speed is of utmost importance, otherwise you'll quickly lose the pus pool bonus. Two taps on the center of the organ should be enough to help you get an initial lock on it. If a hematocele occurs, it's easier to just restart the operation as your hands are full enough with everything else Sige does.
While you're blowing away the gas, smear antibiotic gel everywhere to arrest Sige's movement and keep vitals from plummeting.
Pus clouds left unattended will not proliferate, though they will still create inflammations. Your operating priority should thus be gas wound→exit wound→pus clouds→attack Sige. Inflammations can be largely ignored until Sige is defeated.
